As part of our Social Prescribing (SP) efforts, SCH aims to link patients to their community with the view to improve their Social Determinants of Health.

Volunteer Role:
- Interact with our seniors and befriend them- Assist them in their prescribed social activities (Karaoke, Gardening, Digital Literacy etc) in the communities near their homes- Encourage them to participate in the activities (For those who are visiting for the first time)- Escort our seniors back home after the end of the activity
